The following details how to format a Syquest 44meg cartridge but applies to all SCSI peripherials:
On the SCSI drive the reset enable switch must be ON and the parity switch must be OFF.
From the Disk Utility, select 9 to format the SCSI drive.
Select the drive ID. 0-7 are allowed. 0 is the default for all loads and saves so an extra step is not necessary to select the drive.
Select SPEED when asked to optimize. Either will work, however, the size does not really change much at all and the speed is about 2 - 3 seconds versus 9 - 15 seconds.
Select 2 for interleave
Select YES to "save old defect list". You MUST do this. You cannot get any format to work if you do not select YES to this option.
Select YES to ready and erase
The format does not take too long but the verify of blocks does take a while.
